St. Mary School Athletics are available to St. Mary School and Homeschool students in grades 6-8th. For students to be eligible to participate, all student-athletes need to complete the forms under “St. Mary School Athletic Forms”. In addition, all Homeschool student-athletes need to also complete the forms under “Homeschool Registration Forms”.
St. Mary School Athletic Forms
All 6-8th Grade Students are required to complete the following forms to participate on St. Mary School Athletic Teams:
- Athletic Registration Form
- Sports Physical. If a student is playing more than one sport during the school year, the physical from their pervious sport can reused. Sport Physicals are good for one school year and will need to be completed once per school year.
- Pay the team fee ($125). Full details on how to pay can be found on the Athletic Registration Form. The team fee must be paid for every sport a student-athlete participates on.
Homeschool Registration Forms
For Homeschool Students Only: All 6-8th Grade Homeschool Students are required to complete the “St. Mary School Athletic Forms” in addition to the following forms to participate on St. Mary School Athletic Teams:
- St. Mary School Registration Form
- St. Mary School Medical Form
- St. Mary School Concussion Form
- Pay the school registration fee ($30). Full details on how to pay can be found on the St. Mary School Registration Form. This is a one time fee.
